Use the processes below to notify a managed care organization (MCO) of a KanCare enrollee's pregnancy based on which MCO the enrollee uses.

Sunflower Health Plan
Fax the above form to 1-800-681-5125 or email it to Sunflower.
Sunflower Health Plan encourages providers to complete notification of pregnancy within the portal.
UnitedHealthcare Community Plan
Fax the above form to 1-877-353-6913.
Follow these instructions to complete notification of pregnancy in the UnitedHealthcare Community Plan provider portal:
- Visit UHCProvider.com and log in.
- Select "Clinical and Pharmacy" and then "Care Conductor and Notification of Pregnancy".
- Enter the required information and complete the OBRAF on the last page.
